Introduction: Left internal mammary artery (LIMA/IMA) is considered as the gold standard, in terms of conduit selection during coronary artery bypass grafting (CABG). Harvesting of IMA, as a result takes a significant part during CABG. IMA harvesting during minimal invasive (MICS) procedure is even more crucial.

Methods: in this article we have tried to emphasize how we can harvest IMA with very less effort, minimum injury to the vessel and chest wall, avoidance of clipping, minimum blood loss, minimum spasm etc.

Conclusion: We have tried to show how we can effectively harvest IMA by change of some of harvesting tools, few patient/table positions, use of different endotracheal tubes etc.
